This article provides an overview of types of IDEA-funded and LEA-administered transition programs for older students with intellectual disability, aged 18 – 21, on college campuses. It provides an in-depth description of one program administered by the Baltimore City Public School System, detailing its outcomes. The article also provides recommendations for successful development of such programs, particularly in the areas of interagency collaboration, goals, service planning, work experience, and measurement of outcomes.
