Seamless Transition for Youth with Significant Disability

This two-page brief defines a seamless transition from high school to paid, integrated, supported employment for youth with significant disabilities. It provides a detailed description of the features of this transition for student, family, school system, state vocational rehabilitation and intellectual/developmental disability agencies.  A flowchart lists activities, services and outcomes that should be in place during each of the high school years, the last year of high school, the point of transition and the post-high school years.
