Family Centered Transition Project

This is a two-year collaboration among the University of New Hampshire Institute on Disability, the Center for Community Inclusion and Disability Studies at the University of Maine and public high schools in New Hampshire and Maine to provide transition services to individuals 16-18 who are students in high school and have autism spectrum disorder.  The project trains families in person-centered planning, networking and utilizing adult services and resources, provides planning meetings involving families and actively participating students, and 6 to 8 months of assistance in implementing plans.  This is a federally funded demonstration project with a research design.
