Providing Supports to Youth with Disabilities Transitioning to Adulthood: Case Descriptions from the Youth Transition Demonstration

Article describes three Social Security Administration-initiated Youth Transition Demonstration (YTD) projects that include individualized work-based experiences, youth empowerment, family supports, system linkages, social and health services, SSA work incentive waivers, and benefits counseling.
