Person Centered Planning in a Transition Program for Bronx Youth With Disabilities

Article, based on observation and semi-structured interviewing, describes the implementation of person-centered-planning for youth with disabilities of the Youth Transition Demonstration Project of The City University of New York in a Bronx school district of great disadvantage.  The article considers issues relevant to bringing the program to scale.
