Articulating Roles and Responsibilities in the Transition Planning Process: A Practice Aimed at Figuring Out How to Work Together.

This report describes the steps taken by a regional work group in central New York state to increase the collaboration of schools and Vocational Rehabilitation agencies in transitioning students with disabilities from high school to employment or training.
