An Assessment of Transition Policies and Practices in State Vocational Rehabilitation Agencies

Describes the state and local interagency agreements, identification, referral and application procedures, role of VR personnel, transition services, and resources and funding of transition services in 80 VR agencies.  Describes promising practices in the delivery of transition services to youth with disabilities by VR agencies in Alabama, Colorado and Vermont.
