A Review and Discussion of a Model for Seamless Transition to Adulthood

The Transition Service Integration Model integrates the resources of public schools and rehabilitation and developmental disability systems. Schools and private non-profit agencies work together to develop employment and inclusive community activities during students’ last school year. Rehabilitation and developmental disability systems then work with the agency on an ongoing basis.
