A Guide to Preparing Your Child With a Disability for Life Beyond High School, Minnesota Secondary Transition Toolkit for Families

This guide provides detailed, comprehensive information and encouragement for parents in Minnesota on the process of planning for transition from high school for students with disabilities.  It describes how they as parents can effectively participate in that process.  Of interest, too, is the description of Minnesota’s Individual Interagency Intervention Plan (IIIP) that in Minnesota supplements the IEP to coordinate all services under one plan when the student receives services from at least one public agency in addition to his or her school.
